She's being discharged from hospital. How do I prepare for her return home?
My mum was in hospital and is coming home this week. They say the hospital stay made her more confused. What should I prepare?
It's very common for someone with dementia to leave hospital more confused than when they went in — the strange environment, acute illness, anaesthesia, or the hospital stay itself (known as delirium) can disrupt cognitive balance. A good portion recover in the following weeks, at home, with routine and patience.
Before leaving hospital
Ask for a written copy of: the COMPLETE updated medication list (what's new, what's stopped, what's changed dose), the warning signs to watch out for, and who to call if something goes wrong. Confirm any follow-up appointments already scheduled. If mobility has worsened, ask if there's entitlement to physiotherapy, assistive products (walker, adjustable bed), or home support during recovery.
The first few days at home
Resume old routines as quickly as possible — the same times, the same place at the table, objects in their usual spot. Expect difficult nights and more dependence than before; don't draw definitive conclusions in the first week. Double your attention to falls: post-hospital weakness is treacherous.
If confusion worsens instead of improving, if fever, pain, or a total refusal to eat and drink appears, contact her GP or the health helpline — don't wait for the follow-up appointment.
